The Mine Health and Safety Council presents an award known as the Thousand Fatality-Free Production Shifts Award to the industry for recognition of successful participants.
Eligibility
A mine, or a portion thereof, which is operational and forms a
participating entity as defined by the manager, will be eligible for this award.
Award holder
As soon as a participant records 1 000 consecutive fatality-free production shifts, it becomes holder of the Thousand Fatality-Free Production Shifts Award and retains the award until either:
- Another participant reaches 1 000 shift status; or
- The holder has a fatal accident.
A participant starts the accumulation of fatality-free shifts from the date on which the defined
participating entity is registered with the DME. In the event of the holder of the award having a fatality, the award will be returned to the Inspectorate for safekeeping.
The
calculation of fatality-free production shifts after a fatal accident will commence from the shift immediately following that during which the fatality occurs.
Information to be supplied
Every participant shall supply the required statistical information called for on the prescribed form (Report of Labour and Shifts DME155). The completed form will be sent to the Mine Health and Safety Inspectorate so as to arrive within the month following that to which the information relates.
If the information is not submitted for a period of three consecutive months, the DME has the right to suspend, revert any accumulated production shifts to zero or terminate the participation of the participating entity that has failed to submit such information.
Ten days before the expected date of attainment of one or more thousand consecutive fatality-free production shifts, the statistical department of the Mine Health and Safety Inspectorate will be notified of the expected date of achieving the 1 000 fatality-free production shifts.
Awards decisions rest with DME
Any decision in connection with the awards scheme is vested in the DME. If a participant chooses to appeal against a decision, the matter will be referred to the MHSC.
Transportation of the award
A participant who has become eligible to be holder of the award will, by arrangement with the MHSI, be responsible for the collection of the award from the former holder or from the DME if no other mine has more than 1 000 consecutive fatality-free production shifts to its credit.
Presentation
A participant who has become eligible for presentation of the Thousand Fatality-Free Production Shifts Award will, by arrangement with the Mine Health and Safety Inspectorate, set a suitable date for the presentation of the award. A member of the MHSC should be invited to present the award to the representative nominated by the mine. The recipient will determine the format of the event.
Trophy
A small-scale replica of the trophy will be presented to the successful participant, which will remain the property of that participant. The engraving of the names of all the winners of the floating trophy will be done by the DME.
